Job Description

KEY FUNCTIONS AND RESPONSIBILITIES :

  • Performs direct patient care and executes treatment plans including providing evidence-based patient education, conducting comprehensive sexual histories, STI screening and providing treatment per CCI standing orders. 

  • Care coordination and management for individuals living with chronic conditions such as HIV, Hep C, and TB. 

  • Develops care plans, assesses barriers to care, coordinates transitions of care, ensures access to medication, and supports the referral process. 

  • Maintains a panel of patients receiving PrEP – coordinating follow-up, labs, prescriptions and or administration of injectable PrEP per CCI standing orders. 

  • Supports the clinical and administrative delivery of the Title X Family Planning program, ensuring compliance with program statutes. 

  • Responsible for reporting, budget management, partnerships, and facilitating meetings involved with administering family planning and reproductive health services grants.  

  • Compiles and validates data for annual reports, analyzes patient feedback and chart audits, and supports clinical quality measure development. 

  • Acts as the primary liaison with Sexual and Reproductive Health partners, routinely engaging with teams to implement and audit workflows in primary care and developing onboarding training plans. 

  • Adheres to evidence-based care practice guidelines and CCI standards of care.  

  • Develops and presents SRH educational material as assigned.  

  • Participates in quality assurance, quality improvement, data reporting and workflow analysis projects as assigned. 

  • Consistently seeks avenues for self-education to maintain compliance with evidence-based practice and advancements in sexual and reproductive healthcare.  

  • Maintains the patient’s electronic medical record within expected timeframes, following CCI standards for charting. 

  • Provides quality customer service and communicates in a HIPAA compliant, culturally sensitive, and trauma-informed manner.  

  • Performs other responsibilities as assigned by the Director of Nursing or SRH leadership. 

  • Attends and participates in meetings, committees, health fairs, events and training as assigned.

  • Works within the policies and procedures of CCI Health Services Inc. 

MINIMUM QUALIFICATIONS

  • Active RN or LPN (RN preferred) license issued by the State of Maryland Board of Nursing or any state under the Nurse License Compact.  

  • At least 1 year of clinical nursing experience. 

  • Current BLS/CPR certification required. 

  • Family planning and/or infectious disease experience preferred.  

  • Written and oral fluency in English. 

WORKING CONDITIONS

  • Working on-site is an essential duty for the job – with opportunity for a limited hybrid work schedule.  

  • This position may require travel between CCI sites as needed. 

  • Work under stressful conditions within a controlled, enclosed environment daily. Work environment involves exposure to potentially dangerous materials and situations that require extensive safety precautions and may include the use of protective equipment.

  • OSHA Category 1. Clinical environment. Possibility of exposure to chemicals and infectious body fluids e.g., HPV, HIV, HEP C, HSV.
